Almost the entire rebel assault force is wiped out flying over the surface and flying down the trench, yet Han can fly his ship straight to the exhaust port from deep space, why didn't they all do that?

They certainly would have had a better angle than hoping your torpedoes will make a 90 degree turn.

Also the falcon shoots the TIE fighter next to Vader, and not Vader himself, who was the only one shooting at Luke.

By the time Han arrived, most of the gun turrets were either destroyed or focused on Luke so the Falcon was able to "sneak" up on Vader.

Han had a clearer shot at the TIE next to Vader, knew it would crash into Vader's TIE when he shot it, or something.
