Pretty easy actually. It's a very old trick that I think was first published in S.W Erdnase's Expert of the Card Table in the late 19th century. I think it was Erdnase anyway. I'm probaly wrong about that. It's called the Hummer Whirling Card anyway and is quite popular due to the strength of the effect.
It uses magicans string (very thin but strong basically) attached to the card with wax. It's then set up through the arm and around the body and with elastic loops you're able to wind the card up and then let it go and make it travel around the body. The set up is hung over the thumb so you can make the card seem to go through your hand with "no strings" attached.
It's quite a difficult set-up and the gimmick is quite involved so I've never done it but I have the explanation in an .PDF somewhere around here.
