When rescuing a hostage, the thing not to do is throw a frag grenade into the room?

It is easy to criticise here, as we do not know the "critical information" that led them to attempt the rescue.

But several things shout cock up:
1. The US special forces were sent in, suggesting intelligence from the US was the lead
2. The UK government went public in a rush stating there was an explosion created by the kidnappers - then had to backtrack.
3. Frag Grenade wounds are easy to recognise on bodies, which is perhaps why they had to state what happened.

In addition there have been many rumblings about negotiation and intervention from local tribe leaders as being the best route.

Without a clear idea of what the original red light intelligence was I find it difficult to judge.
 
It's easy to start moaning about more U.S friendly fire but in this case they were trying to rescue a woman in a close quarters situation that was highly volatile.

I do question what the fuck they were doing throwing grenades in to rooms for when they knew that she was probably in one of them but I agree with Headvoid it's hard to judge in this case.

Perhaps they meant to toss in a flash bang grenade but grabbed the wrong one?
 
They are different shapes, sizes, and weights.
 
Saw this on Sickipedia earlier which made me chuckle though..


British rescue operation.

Use stealth tactics to get into the enemy building. Use silenced weapons and gas to disable combatants and avoid detection. Rescue victim quickly and without taking any casualties.

American rescue operation.

Roll up in Hummers, because they are cool. Throw grenades at building until everyone inside is dead, because explosions are fucking cool. Drag corpse of person you just rescued out of the rubble and stick American flag in them. High five anyone in your platoon still alive.
 
Real story:

When I was in the Coast Guard there was a hostage rescue exercise going on at the pier we were tied up to. Across the pier was a buoy tender, and that was the where the "hostages" were, the crew of the tender were acting the part of "terrorists". We were told we could watch and warn the crew of the buoy tender of anything we saw, but we weren't part of the exercise. Special Forces were going to board, remove the hostages, and escape sometime in the next 48 hours.

We all figured there would be helicopters, guys in black, explosions, etc. Everyone wanted to see it so lots of people were hanging out on deck. About two hours later, we were told that the exercise was over. No one saw a THING. According to the crew of the tender, three guys had "You are dead" signs on them, none of them had any idea that the other guys were there until hands grabbed them and put the sign on 'em. This was the crew of this ship, they LIVE on it...and they were completely unable to even raise an alarm until after the exercise. Note: several of the crew did have MLE (Maritime Law Enforcement) experience too.

If they were "pros", they'd get the job done. Something happened. We'll never know.

Well of course something went wrong, the girl wouldn't have ended up dead by a US grenade if everyting went right.
