You can tell by the form and shape. No woman with natural titties can lie on their back with perfect circular tits pointing at the ceiling.
A real tit would be heavier on the side it's leaning towards - teardrop shaped.
The scars can happen in 3 different area from my understanding. Nipple removal + insertion was a crude method. Then under the boob another method was which left an unsightly scar. And the most modern from my understanding is an armpit slit where the scar isn't easily noticeable because the way the skin folds there.
That image is also blurry as fuck on my 4k monitor. You can especially tell by looking at the skin on the thighs. It looks fucking weird where you can see the hip muscle protruding to connect to the femur.
Also, those look like botox lips. But I'm not judging.
