Actually, it was Lois and Lex, but Lana (and Lex) were hospitalised in the previous episode. And D.L. from Heroes turned up.

Karl's ditched being a doctor to join an over 50s stripper troupe and Lou's set to do the same.
 
I definitely haven't seen that old woman (is she Lynne's mum!?) before. I'd remember the accent.
 
I'd forgotten who she was exactly, but you're right. I remember she was Lynn's real mother, but Lynn had been brought up thinking she was her aunt for some reason. She probably hasn't been in it for 3-4 years.

I find her about as annoying as Lou does.
