Tony Hendra first used it in National Lampoon in the 70's. Joke went something like:
John F Kennedy is lying dead on the autopsy table at the Dallas morgue. LBJ is paying his last respects. Suddenly Johnson drops trou and begins fucking Kennedy in the tracheotomy hole in his neck. The doctors, horrified, scream "WHAT ARE YOU DOING??"
LBJ says "Just trying to fool the Warren Commission into thinking it's an exit wound"
